We are located at 19801 Governors Highway, Suite 140, Flossmoor, IL 60422, serving the Chicagoland area, South Suburbs, and Northwest Indiana.

3. Are you accepting new patients?
Yes, we are currently accepting new patients. You can schedule your first visit through our Patient Portal or by calling 708-991-2945.

Appointments & Scheduling

4. How do I schedule an appointment?
Appointments can be scheduled via our online patient portal (Practice Fusion), by calling our office, or by sending a secure text to our direct patient line.

5. Do you offer telemedicine appointments?
Yes. We provide telemedicine visits for eligible patients. These are conducted through a secure platform and can be booked like a regular appointment.

6. What if I need to cancel or reschedule?
Please notify us at least 24 hours in advance to avoid cancellation fees. You can call, text, or use the patient portal to reschedule.

7. What insurance plans do you accept?
We accept most major insurance providers. For a full list, please contact our office. We also offer self-pay and direct care options for uninsured patients.

8. Do you offer payment plans or financing?
Yes. We accept HSA/FSA, Zelle, Cash App, credit/debit cards, and other flexible payment options.
Medical Services

9. What types of services do you provide?
We offer primary care, chronic disease management, FMLA evaluations, pre/post-op care, and VA disability exams through QTC and Loyal Source. See our full list of services here.

10. Do you offer COVID-19 testing?
Yes. COVID-19 testing is available for symptomatic and asymptomatic patients. Results are typically available within 24–72 hours.

11. Can you complete forms like FMLA, school physicals, and return-to-work documents?
Absolutely. These can be completed during your visit, depending on your medical evaluation.

12. Do you have a patient portal?
Yes. Our portal is powered by Practice Fusion and allows patients to access records, request refills, message providers, and schedule appointments securely.

13. How do I get a refill on my medication?
You can request refills through the patient portal, during a visit, or by calling the office. Please allow 24–48 hours for processing.

14. Do you see children or provide pediatric care?
Currently, we provide care for adults and young adults (typically age 18 and older). Pediatric referrals are available upon request.
